Hart Family Practice, PA has been recognized by The National Committee of Quality Assurance as a Patient Centered Medical Home.  We are currently a Blue Cross Blue Shield Blue Quality designated physician’s practice (BQPP).  Our goal is to offer more options for care such as same day appointments and secure electronic messaging between you and your provider. We use technology such as electronic medical records, and a patient portal to communicate and coordinate your care. You receive care from a team of medical professionals led by a primary care provider. You have a vital role in your care by being active in your medical decisions. 

Instead of the traditional "fee-for-service" model where doctors are paid for the volume of tests or visits they perform, an ACO incentivizes doctors to focus on overall patient health and care coordination.
